Impact 24™ is our accelerated start-of-care program for urgent needs, designed to begin services quickly and safely. Services may start within 24 hours after an in-person nurse assessment, based on availability and clinical appropriateness.
What IMPACT 24™ Includes
Priority Intake Review
Impact 24™ referrals are reviewed immediately so our team can identify needs, risks, and next steps.
Registered Nurse Assessment (Expedited)
A Registered Nurse completes the in-person assessment as quickly as scheduling allows, prioritizing clinical urgency and client safety. This assessment establishes the care plan and determines what is needed for services to begin.
Care Start Within 24 Hours of Assessment
Once the Registered Nurse assessment is completed, clients are scheduled to begin services within 24 hours, based on availability and clinical appropriateness.
